Autoimmune liver diseases (AILDs), including AIH and PBC, present significant challenges in Latin America due to delayed diagnosis, advanced disease at presentation, and access barriers to treatments and transplantation. High incidence of AIH-related liver transplantation (25%–30% in Argentina) and advanced PBC stages (32%–78% cirrhosis at diagnosis) highlight regional disparities. Genetic variations, such as HLA-DR13 association with better outcomes in AIH, and diagnostic limitations, including low AMA-negative PBC testing availability, are critical. Treatment gaps include inconsistent second-line therapy access and socioeconomic barriers affecting adherence.
